You fired him through the Actions menu on the Front Office -> Personnel tab, right?

The double “fired” is a thing OOTP does, I wouldn’t worry too much about it.
No I didn’t fire the BC. I fired the AGM after the BC was fired because I thought it was the AGM that fired him. But his firing never showed up. Also, tried to hire a new BC and no offer shows up. It’s just odd stuff.
The AI would never fire a bench coach mid-season, I don’t think it’s that.
If you make an offer to a new BC I don’t think it shows up anywhere. But you can verify by right clicking on the BC – there should no longer be a “Make an offer” option because you’ve already made one.
This is pretty confusing. So, your bench coach was fired, but not by you. And you tried to fire your AGM but it didn’t take?

there is a screen to see Pending Offers. also check the mail, maybe there is a note from the guy or from the AGM ?

There’s nothing in the mail, but there had been. There was an announcement on Blake’s firing in which he spoke of leaving a sinking ship. Poof, it must have gone away with his dismissal.
